Pancreatic Ductal Adenocarcinoma (PDA) remains an almost incurable tumor with a 5-year overall survival rate of 9%. Surgical resection is the main treatment to achieve a long-term survival since PDA is mostly resistant to chemotherapy (CT). The main reason for chemoresistance is the desmoplastic compositions of tumor microenvironment typical of PDA. Moreover, PDA tumor microenvironment is poorly populated by leucocyte cells and in advanced cancer most of them are represented by myeloid immunosuppressive cells and regulatory T cells (Treg). Many studies exploited the treatment with immune checkpoint blockade (ICB) in PDA, since important results were obtained in other types of cancer in terms of increasing survival depending on a strong induction of antitumor T cell response; however clinical trials with both anti-PDL1 and anti-CTLA4 were not able to substantially increase the survival of PDA patients, therefore the need to find new strategies is increasingly urgent.
